Fourth Street Clinic helps individuals experiencing homelessness improve their health and quality of life by providing high-quality integrated health care.
For many homeless Utahns, Fourth Street Clinic is their first and only chance at a diagnosis and on-going treatment. By increasing homeless Utahns access to primary care, Fourth Street Clinic is a major partner in ending homelessness, promoting community health, and achieving across-the-board health care savings.
Fourth Street Clinic provides extensive health care services to its patients, something that is unparalleled in the state of Utah. Here are some of the clinic services offered on-site to our patients:

- Care management and coordination

LGBTQIA+ RESOURCES
Fourth Street Clinic welcomes, accepts and provides care for everyone, regardless of race, gender identity or sexual orientation:
- WPATH mental health letters for surgery
- Hormone therapy coordination
- Gender affirming surgery referrals
- Respite housing for surgery recovery
- Medicaid enrollment services
- Case Management assistance for legal name and gender change (for Utah residents)
Weekly Wellness Classes:
Making lifestyle changes to overcome chronic disease, addiction or mental illness requires knowledge, skills and long-term encouragement. You do not have to do it alone and you will have more success with support. Fourth Street Clinic offers several healthy lifestyle classes to give you the tools you need for success.
Wellness classes are free to attend. Talk to your provider to see if any of these classes are right for you.
Check-in time is 2:30pm. All classes are from 3pm – 4pm.
